• What is collected: your name, your mobile number, your preferred language, whether you have seen the doctor before, and anything you choose to write in the reason and preferred times fields.
• Why: only to contact you and arrange an appointment.
• Where it goes: into the clinic's confidential medical record system, on a private server.
• Who sees it: the doctor and clinic staff. It is not sold, and it is not shared for advertising.
• How long: it is kept as part of your clinical record.
• Technical note: the web server records the network address your request came from, to detect abuse of the form.

Your choices

You can ask the clinic to correct or delete what was sent, by telling the doctor or clinic staff at your appointment or when the clinic contacts you.

If you do not agree, please do not use the form, and contact the clinic another way.
